2. use of Foundation Church campus

(Must be ministry sponsored.)

Event Announcement

Please use this form to have your event promoted within the church (bulletin, inserts, pulpit etc.)

This is in addition to the Ministry Events Form, which must also be filled out to secure use of our campus.

Please use this form to request either 1) reimbursement, or 2) payment request. Must have prior approval from the ministry or department leader.
